The Chautauqua Center (TCC) is an ever-evolving Community Health Center, nearby the Canadian border, city of Buffalo, and PA state line. Recently, we designed a mobile unit for care, offer veggie prescriptions for patients, hired an OBGYN, and opened an in-house pharmacy. Both of our offices include a community kitchen and a health and wellness program for staff and community members. TCC serves all in the community, 40% of our patient population from the Latino community, and provides care in the following areas: primary care, dental, behavioral health, substance abuse counseling, pharmacy, nutrition, chiropractic care, women's health/family planning, pediatric care, OT/PT/Speech Therapy, podiatry, tele-rheumatology, and care coordination. Why TCC? Providers have joined as there's a "healthy" balance for activities and also enjoying the simple pleasures for single individuals and families who relocate to the area. We have also seen an increase of those who appreciate more of a lower cost of living area, such as Chautauqua County. Providers value the manageable caseload of patients, weekends off, as well as feasible on-call schedules, malpractice including tail coverage and many other benefits. Staff have joined us from Buffalo, Ohio, UT, California, Missouri, Canada, North Carolina, PA and some choose to work 4-day work weeks. Purpose: Diagnose and treat problems with a patient’s teeth, gums, and related parts of the mouth. Provide advice and instruction on taking care of teeth and gums and on diet choices that affect oral health.

Education/Skills/Qualifications:

• Licensed to work in the state of New York as a Dentist

• Graduate of an accredited college of Dental Medicine

• CPR certified

• Knowledge of current principles, methods and procedures for the delivery of medical evaluation, diagnosis and treatment in area of expertise

• Minimum 1 year experience in a primary care clinic

• Legal authorization to work in the United States

• A valid State Driver’s license with clean records and access to insured automobile

• Excellent interpersonal and communication skills required

• Leadership and organizational skills essential

• Bilingual especially in Spanish preferred

• Computer literate preferred

• Self-motivated and flexible to daily patient schedule and/or clinic needs
